Big Weekend away Consent questions:  
I am aware that childrens activities will be run in line with HBC Chester safeguarding policy and that youth/childrens activities may be delivered by people/organisation outside of HBC Chester.  
I have read all the above statements and agree to them to, I also declare I am the parent/guardian of child listed on this form.  I declare all information listed is correct, and will contact you if any information changes.  I give consent for my child person to attend child activities.  
Children photograph consent

Photographs and video are sometimes taken during the Big Weekend away.
Images are used: 
  • In videos or slideshows for use in church events (e.g. Sunday services) for publicity purposes, including on the church website and related social media HBC Chester and Overflow Church are committed to safe practice when dealing with images of children.
  • No names or any other personal information, which could enable identification of a child, are used.
  • The use of images is subject to the HBC and Overflow Church, Safeguarding Policy and relevant Individual Group Policies, which provide safeguards against misuse of the images. Images are securely stored and password- protected, subject to the legal requirements of the Data Protection Act (1998) and in accordance to our GDPR policy.
